For those families who have students who will be participating in athletics at Carbon High School, there will be a free athletic physical night scheduled for Wednesday, July 10 from 5:30 - 7:00 PM. The event will be taking place at Dr. Kelly Jensen's office at 280 Hospital Drive in Suite 3.
Please do not call the Doctor's office to schedule an appointment for this event as this is after hours for the doctors and their staff. It is a free service that does not require an appointment.
Please print off the physical packet attached below and fill out page 2, and all of page 4 and bring the entire packet to the physical. Those documents will be collected by the athletic director the night of the event and uploaded into your student's Aktivate account.
For those of you who may not know Register My Athlete is now Aktivate and the web address is www.aktivate.com. Nothing has changed except for the name.
